All membership fees are annual based on original enrollment date. Changes to membership packages may be prorated during the year.
Payment Options
Cash, check, credit card, and payroll deduction are all options for payment of membership fees.
In the case of payroll deduction, fees will be deducted from 24 consecutive paychecks. The number and amount of deductions will be prorated for employees with less than 12 month positions.
The following groups allow payroll deduction: West Virginia University, University Health Associates, WVU Foundation, and the WVU Research Corporation.
Documentation
Marriage certificates (spouses) and birth certificates (dependents) are mandatory when purchasing a family membership.
